Case Studies
Two to three case studies will be presented by a group of students each week.
The presentation should be max. 7 minutes (I will use a timer)
The presentation of the accident should address the following questions:
- What is the chain of event and what are the causes for the incident?
- Which barriers failed (technical, organizational?)
- What is the origin for the released energy (the consequences)?
- What appropriate risk reducing measures (barriers) would be required to restart the production? How would they hinder the chain of event of the incident? What type (technical, organizational) of barriers are those?
- What are the main learnings from the accident
