train ticket reibursement form

Description
The train tickets will be reimbursed individually.
1) Complete the form uploaded here (the 4 empty sections: name, address, total amount, bank details & iban)
2) Save it as a PDF file
3) Add a photo of (1) ID, (2) bank card (with the IBAN), and (3) the expense receipts (should they not have a card with the iban on, any document from the bank where it is written)
All in that order and in a single PDF document. Any document which is not precisely in that format will be refused and not reimboursed.
The reimbursement will then be initiated on their personal account, within approximately 2 weeks after reception of this document.
The single PDF document must be sent to the section (karine.brahimi@epfl.ch)
